Position Summary

RHD is seeking Full Time LPNs in the Monroe area. Shifts available are Sunday to Wednesday 7a-3p, Sunday to Wednesday 3p-11p, Sunday to Wednesday 11p-7a, Thursday to Saturday 7a-7p and Thursday to Saturday 7p-7a. The primary role of a LPN is the oversight, coordination and direct intervention of any and all health-related issues directly or indirectly related to any and all participants of the program.

Essential Duties and Functions
  • Maintains nursing documentation and medication records
  • Assists residents in learning about their medications; assures that medications are taken as physician ordered and recorded in electronic MAR
  • Provides nursing care including, but not limited to, wound care, trach care, diabetic counseling, etc
  • Assesses residents' health and emotional needs
  • Communicates with other members of the nursing and program staff; communicates serious issues to Nurse Manager and Program Director
  • Maintains confidentiality, including keeping voice low and not talking about residents in public areas
  • Other duties as assigned
Qualifications
  • Licensed Practical Nurse with current license

  • Medium: exerting up to 50 pounds of force occasionally, and/or up to 20 pounds of force frequently, and/or up to 10 pounds constantly to move objects.
Reporting Relationships
  • Direct Supervisor: Unit Director

RHD Connecticut provides residential community living arrangements, day services, and individual rehabilitative supports to people who have developmental disabilities in the West and South regions in Connecticut. Currently RHD CT operates 10 CLAs, Continuous Residential Support programs and supports individuals who reside in apartments in the community. RHD CT operates one day program e the South region of the State (Hamden).
