Position Summary: The Assessment & Referral Counselor is responsible for facilitating facility admissions by performing initial assessment and referral activities for all in-coming and potential patients, consulting with potential patients, patient families, medical staff and other hospital staff, and counseling patients and patient families with regard to current assessments, recommended referrals and recommended required levels of care.
