Posting Date October 4, 2021 Job Title Scheduler Home Care Full Time Specialty Hours FT Overview Position Description: The Scheduler will be responsible for managing/assisting in the daily schedule for the Home Care team. Assigns case managers to each admission, resolves issues should they arise for the continuality of services. Triages staff/patient calls and assists in resolving scheduling issues that may occur. Completes EMR scheduling workflows with accuracy and within agency guidelines. Completes weekend schedule for nursing and rehab staff along with weekend communication report timely and with accuracy.
