Roze Room Hospice is seeking a full time Social Worker (MSW) to meetwith hospice patients and families to explain the hospiceservices/benefits.

This is a field position.

Coverage areas include Los Angeles/South Bay areas.

$30-35/hr + Mileage Reimbursement (based on experience)

  • POSITION SUMMARY:

    The Hospice Counselor is a qualified Social Worker/Counselor whoengages in community outreach to counsel, explain and educate patients andfamilies about hospice and palliative care services; reviews consents and otherdocuments and assists with election of hospice benefit. The HealthCare Coordinator coordinates patient discharge from hospital into hospice orpalliative care programs. The Hospice Counselor works cooperatively in coordination with the hospice and/or palliative careteams. The Hospice Counselor requires a desireto work directly with patients and families to assist them with seamlessly andefficiently accessing hospice and/or palliative care services. Must be able to utilize computers and becomfortable with electronic medical record. Must be able to meet with patientsand families in person and/or remotely.

    QUALIFICATIONS:

    • Degree in social work preferred
    • At least two years of health care experience preferred
    • Knowledge of hospice requirements including Medicare, Medi-Cal, and private insurance guidelines. Ability to communicate effectively verbally and in writing. Ability to connect with patients, families and community
    • Valid driver's license, and auto insurance, reliable/well maintained transportation
