The Community Support Specialist provides integrated, holistic, community-based support services.
ESSENTIAL FUNCTIONS:
Provide services to address activities of daily living, including the management of symptoms associated with psychiatric, physical, and/or substance use disorders per program standards and the 8 Dimensions of WellnessComplete services consistent with the requirements outlined in the expectations worksheet and program manual with the supporting documentation completed in a timely manner (within 72 hours)Maintain all assigned client medical records to program standardsProvide referrals, advocacy, information and education to meet individual needs of clientsProvide appropriate risk assessments and facilitate access to needed interventions as indicatedProvide transportation for clients as neededMonitor client hospitalization and participate in discharge planning and follow per program standards
