Pay Range*: $20.50/hr - $28.08/hr

Position Summary

The Community Health Worker is responsible for functioning as a member of a multidisciplinary healthcare team in the field to consistently promote patient advocacy through outreach and coordination of services.

Essential Functions and Responsibilities of the Job

1.Conducts home visits and visits each family as required to meet program goals.Educates and advocates for parents of children.Helps families get appropriate health care services.Ensures proper and timely follow-up care with health care providers.Assesses additional needs and facilitates referrals and enrollment in community support services.

2.Assesses the home environment, including situations that may lead to child abuse/neglect, developmental delay, or social isolation. Communicates concerns and issues timely and appropriately.

3.Serves as a resource for schools and community organizations.Provides education and reinforcement, both one-on-one and in groups.Maintains contact with providers and medical staff.Helps improve communication with child's school and/or childcare, and acts as a liaison between the families and medical care providers.

4.Maintains accurate patient files and program documentation. Follows-up with clients: keeps ongoing contact with families as required to meet program goals.Attends scheduled department meetings and gives accurate monthly progress reports.Maintains daily schedule and keeps calendar up-to-date.

5.Attends ongoing educational training, scheduled group meetings and other activities, including several community based organization meetings.Participates in community health fairs and special events.Provides peer support.

6.Provides interpreter services in the field to assist with coordination of care.

7.Acts as a liaison for system and patients referred for care and follow-up. Assist program coordinator with required program data entry and statistics as needed. Maintain a library of community resources relative to social and healthcare services as a resource for staff.

8.Demonstrates understanding of age specific competencies for the infant, child, and adolescent population.

9."Attends department specific staff meetings, in-services, education, and training. Participates in selected department specific process improvement projects. Participates in the orientation and development of students and/or new staff as assigned.

10.Performs other duties as assigned.

*Placement in the pay range is based on multiple factors including, but not limited to, relevant years of experience and qualifications. Qualifications

Minimum Requirements

Qualifications/Work Experience:

2+ years experience in the health care field, Pediatric experience preferred.

Must be culturally and linguistically competent and sensitive to the target community.

Knowledge of community resources preferred.

Able to work with individuals, families and community groups to promote healthy lifestyles.

Basic computer skills in Microsoft Windows environment including Word, Excel and Power Point.

Excellent interpersonal and communication skills, verbal and written required.

Bilingual English/Spanish preferred.

Education/Licensure/Certification:

High School diploma or GED. Requires the use of personal vehicle, must have California Driver's License and proof of auto insurance.
