Registered Nurse (RN) (Hospice)

OSPTA Hospice is seeking full time Licensed Registered Nurse to provide services to hospice patients in Uniontown and surrounding areas.

* 1 yr of RN experience required
* Hospice experience preferred
* On-Call as needed backup
* Must be willing to travel in all weather conditions
* Strong computer and technology skills
* Excellent organizational and communication skills
* Ability to work independently and within a multidisciplinary team
* Active patient contact within past three years, preferred.

*RN Qualifications and Skills: *Graduate of an approved registered nursing program; licensed to practice, in the state in which they practice; have good oral and written communication skills; be of excellent health and possess a high energy level; demonstrate a commitment to excellent patient care and customer service.

*Knowledge/Skills/Ability:* Ability to work independently, make accurate, and at times, quick judgments. Ability to supervise others appropriately. Ability to respond appropriately to crisis outside of a hospital setting. Acceptance of and adaptability to different social, racial, cultural and religious modes. Completes Hospice training program.

*Essential Functions:*
* Regularly assesses and reassesses the nursing needs of the Hospice patient.
* Provides dietary counseling.
* Provides Hospice nursing services, treatments and preventive procedures.
* Initiates nursing procedures appropriate for the patients Hospice care and safety.
* Observes signs and symptoms and reports to the physician and IDG members any unexpected changes in the patients physical or emotional condition.
* Teaches, supervises and counsels the Hospice patient and family members about providing care for the patient.
* Supervises and trains other nursing service personnel.
* Develops and re-evaluates the patient/family care plan in conjunction with IDG to meet needs and maintain continuity of care.
* Performs specific nursing procedures as needed (e.g., treatments, management of symptoms) following doctors orders.
* Maintains records as required by Hospice.
* Coordinates the implementation of the plan of care for patients residing in SNF, NF, ICF or MR.

Salary will be based upon qualifications and job requirements. Signing bonus $5000 after 90 days.

Pay: $80,000.00 - $100,000.00 per year
