Overview:
Salary $45,000 year
Scope of Position
- Provide support and assistance to the Direct Care Worker Job Quality Grant program participants areas related to completing educational programs, obtaining/retaining employment and career advancement. Develop external and internal talent pipelines to mitigate staffing shortages in the healthcare field.
Responsibilities:
Essential Function
- Explain and determine eligibility to participate in the Direct Care Worker Quality Grant program, obtain required documentation, complete intake application that notes demographic information required for grant compliance.
- Coordinate outreach and partnerships with educational and training providers.
- Provide support that participants need to overcome barriers and reach their employment/educational goals.
- Provide linkages and established referral systems with community partners.
- Provide guidance and counseling on an individualized basis to program participants to address their needs and barriers.
- Document all activities provided under this grant including eligibility, services provided, support and outcomes.
- Monitor participant progress via outreach, at least monthly, in order to ensure successful education and training outcomes are being met.
- Offer follow-up support services to help participants retain employment or educational goals to ensure that performance measures are met.
- Assist in the outreach and recruitment of new participants.
- Ensure the required enrollment paperwork and documentation is completed and located in the participant file, document all services, progress reports and completion reports in CWDS.
- Maintain data on education, employment, retention and other performance measure requirements.
- Attend all staff meetings in order to contribute to program success.
- Attend scheduled in-service training in order to develop professional skills.
- Complete program participation pre and post evaluation survey on program participants.

Qualifications:
Education and Experience Requirements
- Ability to prioritize tasks and document visits and contacts
- Demonstrated telephone, computer and written communication skills
- Ability to maintain flexibility as needs of contract or contractor require
- Bachelors degree or related in human services, business, education, training or related area is preferred
- Previous customer service experience required
- Previous experience working in a healthcare setting is preferred
- Working knowledge of MS Windows, Word, and Excel
- Valid Drivers License & clean driving record required
