Job Description:

POSITION SUMMARY

The Case Management Assistant (CMA) is a member of the Case Management staff and provides clerical support for the department, secures discharge planning needs as directed, and works in collaboration with Case Managers. Serves as a link to community resources, connecting patients to services needed for aftercare or continued care and facilitates authorizations when delegated from the Case Manager, obtaining measurements from Wound Care nurses, authorizations for wound vac and/or similar products and obtaining Durable Medical Equipment (DME) authorizations. This position assists with the implementation of the discharge plan as directed by the Case Manager and assures all discharge needs are aligned as requested. Provides excellent customer service, thrives in a fast paced environment and contributes as a resourceful problem-solver on the Case Management team.

POSITION QUALIFICATIONS

Education/Licensure/Certification:

* High School graduate or equivalent required.

* Basic medical terminology preferred.

* A minimum of 2 years of secretarial experience in a healthcare setting strongly preferred.

Knowledge:

* Knowledge of records maintenance and retrieval systems.

* Strong customer service experience required.

* Knowledge of the general flow of a patient admission and discharge process required.

* Experience accessing Epic and other hospital computer programs preferred.

* Good understanding of Health care Financial Systems, including contracting limitations with insurers and aftercare providers.

* Familiarity of insurance carriers and the authorization process preferred.

Skills:

* Proficiency in hospital wide computer programs, Windows based word processing, spreadsheet (Excel), email and calendaring software required.

* Must have excellent interpersonal skills in order to interact effectively with others in various situations.

* Must have the ability to work with minimal supervision; frequently reprioritize workload and handle multiple tasks at once.

* Must have the ability to establish a positive rapport with patients and the public.

* Problem solver that works in close collaboration with the Case Managers.

* Basic understanding of hospital computer systems utilized.

* Must have excellent organization skills.
