Hours Per Week: 40 Hour/ Week

Work Schedule: 8:00 AM to 4:30 PM

Summary:

The Patient Navigator is responsible for providing health education targeting patients' needs and health risk factors, assisting patients to establish health goals, monitoring patient health outcomes, and serving as a key referral source for patients diagnosed with cancer or identified at high risk for cancer. The Patient Navigator is responsible for helping patients and their families navigate the healthcare system and community-based services by eliminating barriers to care. This is a grant funded position. The Patient Navigator will ensure work aligns with and meets all grant guidelines and deliverables.

Responsibilities:

  • Provide health resources and education to referred patients focused on health risk factors.
  • Partner with patients and their families to help navigate the healthcare system and community-based services that target their health and social needs and/or eliminate barriers to care.
  • Serve as a patient advocate and liaison.
  • Provide financial counseling and education to patients who identify cost as a barrier to care.
  • Continuously expand knowledge and understanding of community resources, services and programs within the community.
  • Build partnerships with healthcare and community resource providers.
  • Build patients understanding and skills to effectively communicate with healthcare providers and organizations.
  • Responsible for meeting reporting requirements.
  • Ensures compliance with funder guidelines.
  • Demonstrates ability to build and maintain strong working relationships with community partners.
  • Maintains a safe environment which includes knowing and understanding hospital and departmental policies and procedures.
  • Report and directly address identified environmental hazards when appropriate.
  • Report and directly address violations of patient safety policy and/or protocol when involved or observed.
  • Other duties as requested by Director within the scope of the grant.
