RN Case Managers are an essential part in quality care and coordination for our patients. Responsibilities:
* Oversee the implementation and ongoing assessment of the patient s plan of care
* Work closely with the IDG team (spiritual care coordinator, social worker, hospice aide and medical director) to ensure proper coordination and needs for patients and families.
* Ensure an accurate evaluation and work to develop and implement a comprehensive care plan that addresses patient needs.
* Communicate patient conditions and collaborate with appropriate providers to deliver care when patient needs evolve.
* Actively participate in IDG meetings.
* Provide supervision of aides and nurses.

Requirements
* Licensure: Current license to practice as a Registered Nurse (RN) in the State of Pa.
* Current CPR/FA certification.
* Valid state-issued driver s license, automobile insurance, and reliable transportation; willingness to travel.
* One (1) year of professional experience practicing as a Registered Nurse (RN) in a hospice or similar setting.

*Advanced is willing to provide Employees with three (3) options in relation to a Sign-On Bonus. Those being a one-year, two-year and three-year option. Advanced agrees to pay Employee a Sign-On Bonus of up to a period of three (3) years, subject to the Repayment provision.*
* *1 year - $5,000*
* *2 year - $10,000*
* *3 year - $15,000*

Job Type: Full-time

Pay: $65,000.00 - $85,000.00 per year
