Description: Essential Functions:As part of the hospice team and under the supervision of the supervisor or designee, this person works in the office to support the hospice team and contributes to patient care by:Triage phone calls from patients to provide routine information to callers and/or routes to the appropriate individual or relays messages - Answers phone calls from a variety of sources including a potential for being first person to answer a death call from a crying family member.Providing clerical support to the intake desk, clinical and administrative staffCoordinating and maintaining scheduling records and staff timecards as definedAssigning patients to appropriate field clinician based on departmental policy, maintaining lists of patients assigned to each case manager or skilled disciplineCoordinating and maintaining scheduling records as operationally definedData entering patient referrals into the system for appropriate placement, tracking for billing purposes and other data entry for as operationally definedContacting patient or representative as appropriate to determine date/time of planned visit as appropriateCommunicating effectively with field clinicians using a variety of communication tools (phone/written/verbal/fax) regarding assignments and changes per departmental policyMaintaining clear communications between hospice department and medical center staff, patients, families and community agencies as indicatedParticipating in quality assurance activities of the department as appropriate Copying, faxing, filing appropriate documents, and distributing them appropriately per departmental policyReviews, distributes department mailManages forms, letters, reports and correspondence including confidential correspondence for membership accounting, billing, documentation and order processing, and preparing agendas for meetingsMay keep records of operational information, including time cards, post information to records and logs and maintain administrative filesPerforming assigned duties in a timely and accurate mannerAssures all clerk duties are covered daily in order to meet operational needs and department work flowsAdditional duties may be assigned due to operational need per bargaining guidelinesHours may very or include weekends due to operational need per bargaining guidelinesGrade 04 Basic Qualifications High School Diploma or equivalent; business school/college level training preferred One-year medical office experience required Two years of secretarial/office experience within the past five years required Knowledge of medical terminology and health care environment required Typing Speed of 35 wpm or 6000kph required Ability to demonstrate proficiency with various basic computer applications (Office, Word, Excel) and common office equipment (fax/copier/pc, etc) required Ability to learn and be proficient at Health Connect, tapestry, Allscripts, one link, time card entry and any other related systems used within the department Appropriate customer service skills required to deal effectively and courteously with a diverse group of internal callers and visitors as well as occasional external contacts on the phone and in person Must be organized and able to prioritize sensitive work routines Must be flexible, able to work well with frequent interruptions and willing to perform different tasks within an office setting as operationally defined Demonstrates excellent interpersonal skills which enables the individual to work with a variety of personalities and negotiate sensitive situations regarding scheduling of patient visits with field clinicians Demonstrates strong problem-solving skills allowing the individual to assist in offering solutions to patient/clinician scheduling issues under the direction of management Detail oriented to ensure that discussions with staff and patients as well as decisions regarding scheduling patient visits are documented appropriately Previous experience with scheduling of patients visits in a home health setting strongly preferred Knowledge of home health agency operations, Medicare and Medi-cal preferred Must be able to work in a Labor/Management Partnership environment.
