Overview
Responsibilities:
* Join an established Primary Care practice
* Perform patient assessment and assign diagnosis based on recorded assessment data and current scientific and current research.
* Establish plan of care and implement interventions in accordance with care paths, protocols, standards of care and current research.
* Provide education and counseling to patients, families and groups.
* Clinic hours are Monday - Friday 8am-5pm

Qualifications
REQUIRED QUALIFICATIONS
* Master's degree or higher in nursing or physician assistant studies from an accredited program
* Current Illinois license as a nurse practitioner or physician assistant
* Certification by the American Nurses Credentialing Center (ANCC) or the National Commission on Certification of Physician Assistants (NCCPA)
* Minimum of two years of experience as an APP in primary care or a related field is preferred
* Current Basic Life Support (BLS) and Advanced Cardiac Life Support (ACLS) certifications
* Proficiency in electronic medical record (EMR) systems
