Job Description The CTC is responsible for providing the consistency and predictability necessary to transition a patients care between care settings or levels of care within a care setting in a timely fashion. Positions herself/himself as a resource representing the benefits that Heartland provides in the market to a limited number of assigned accounts. Once a referral has been made to Heartland, the CTC coordinates the care transition for patients requiring the services of any of the products offered in that market including hospice, home care, infusion and IPU services as applicable. The role may include explaining benefits, gathering information towards the determination of eligibility, completing part of the admission process and providing patient care visits as appropriate. The CTC partners with the sales team to develop and maintain referral source relationships.

**General Responsibilities**

* Meets with potential patients who have been referred to Heartland to discuss home health care, hospice and/or I.V. Care services.

* Advocates on behalf of patients to ensure receipt of entitled benefits.

* Gathers information regarding the patients qualification for care.

* Visits Heartland patients hospitalized during an episode of care (home care, hospice and IV care), and communicates with the DPS regarding the patients status.

* Meets with the patient and family to begin gathering the care needs the patient is likely to have when transitioning back home.

* Arranges for supplies and equipment needs and communicates with the DPS or designee to safely transition to the next care environment.

* Coordinates with the Heartland admissions team to schedule the admission and transition the care in collaboration with the Heartland Nurse Case Manager, and collaborates with referral sources to arrange for supplies.

* Updates referral sources on status of patients currently receiving service, non-admits or recently discharged and the patients response to services received.

* Provides training and education to assigned referral sources.

* Partners with other members of the sales and operations team to develop and execute account development plans to grow the business relationship.

* Participates in the growth meeting, staff meeting and appropriate sales and clinical training sessions.

* Effectively uses IMPACT in account and relationship development.

* Documents schedule and relationship development activity using CRM daily.

* Participates in agency quality assessment performance improvement (QAPI) programs.

* Adheres to work schedules in completing and performing assigned tasks.

* Applies the policies and procedures of the agency and the rules and regulations of Federal and State regulatory agencies in performing the referral and admission processes.

* Ensures growth and profitability of the company through the responsible use of company resources.

* Accepts other assignments as appropriate.

* Adheres to established personnel policies.

Job Requirements **Education**

* High School Diploma or equivalent required.

**Qualifications**

* Excellent written and verbal communication skills. Excellent interpersonal skills.

* Ability to work well with an interdisciplinary team and health care providers.

* Ability to establish and maintain rapport with clients and referral sources.

**Experience**

* Minimum of two years of experience delivering home health care, IV care or hospice services.

* Demonstrated knowledge of payer sources.

* Demonstrated knowledge of the referral source types and community resources.
