Description

Planned Parenthood of Greater Ohio is seeking a mission-minded Associates to join us as a full time (37.5 per week) Patient Navigator.

SUMMARY
The Patient Navigator provides case management and care coordination services to the patients of PPGOH, by collaborating with internal and external teams that provide healthcare and non-health services within the medical neighborhood. This would include but not be limited to PPGOH health center staff and providers, customer contact center, centralized follow-up, billing, education and outreach, as well as primary care providers, hospitals, behavioral health providers, other specialists such as OB/GYN Specialists, community organizations, and insurance/payers. The Patient Navigator supports the PPGOH Care Coordination team to continuously build out and identify patient needs in the medical neighborhood to ensure our ability to provide wrap around care. The Patient Navigator also supports community outreach efforts in collaboration with the Director of Care Coordination and Referral Resource Manager. The Patient Navigator contributes to PPGOH's mission of providing high quality, equitable, and patient-centered care, as well as creating an outstanding patient experience. Additionally, the Patient Navigator ensures the organization is compliant with fulfilling these duties in line with the parameters set forth in the organization's Medical Standards and Guidelines, PPGOH policies and procedures, and any other regulatory requirements.

WHAT YOU WILL DO

  • Navigates a patient's care with a person-centered and team-based approach with a health equity lens that centers health disparities.
  • Provides case management to patients with healthcare needs including but not limited to: OB/GYN services, Imaging services, Primary Care, Hypertension, Diabetes and acute and chronic disease management, GAC, Tobacco Cessation, Behavioral Health, and PrEP and PEP services.
  • Systematically prioritizes case management by equitably identifying and directing resources based on need to prevent poor outcomes.
  • Connects patients to resources and services in the community to address social determinants of health.
  • Assists patients with navigating insurance coverage and benefits included in Medicaid, Medicare, and Private insurance plans.
  • Identifies and connects patients to financial assistance and resources needed, and helps patients complete documents required to obtain financial assistance for medications and services needed.
  • Processes and resolves prior authorizations and follows up with provider for denials.
  • Triages and assists providers with managing emergent/urgent patient needs.
  • Appropriately communicates to external providers and resource contacts for care transitions to ensure continuity and seamless navigation to services and resources needed by the patient.
  • Collaborates and supports the Referral Resource Manager with identifying and managing referral resources for patients to continuously strengthen the medical neighborhood serving PPGOH patients.
  • Works collaboratively with the Centralized Follow-up Team, Clinical Quality Managers, Training Team, Providers, and Health Services Staff to meet patient needs.
  • Makes a connection and maintains communication with health center staff and providers to build trust and ensure they have the appropriate resources and understanding of internal process to connect patients to a navigator/resource.
  • Works collaboratively with all PPGOH's Patient Navigators, including Abortion Services, to ensure sustainable and timely services are provided to our patients.
  • Works with administration to develop and sustain systems to support PPGOH patients.
  • Acts as an advocate for patients in the GAC program.
  • Accurately enters appropriate information into the electronic health record system.
  • Schedules appointments for patients as needed.
  • Conducts high-level educational group meetings and training for staff.
  • Serves as a resource in the orientation and annual training of health center staff.
  • Demonstrate Planned Parenthood's In This Together ethos by ensuring that the people who make our mission possible are supported, cared for and engaged.
  • Participates in PPGOH Patient and Employee Experience Programming for continuous improvement efforts and assists with goal setting and initiatives, particularly around DEI/REI efforts.

WHAT YOU WILL BRING

  • Bachelor's degree in social work or related field preferred, or equivalent work experience
  • Previous work experience with transgender patients, at least 3 years preferred
  • Experience conducting trainings, particularly on through a health equity lens
  • Prior healthcare experience preferred
  • Bilingual skills/bicultural knowledge preferred
  • Ability to establish rapport among a variety of internal and external constituents.
  • Ability to lead with a respectful, positive regard for employees and volunteers.
  • Commitment to Planned Parenthood's mission, pro-choice philosophy, goals, and objectives necessary.
  • Demonstrate a positive image of organization and self
  • Self-starting individual with proven ability to take ownership for results and driving change
