Job Summary: The nurse navigator is a key member of the Heart Failure Team, providing for time-sensitive cardiac nursing expertise followed by continued clinical oversight and care coordination for patients admitted with a diagnosis of heart failure within the hospital. This position assists in the execution and evaluation of evidence-based processes to achieve the goal of excellent care and expected outcomes.

Minimum Education: Has completed the basic professional curricula of a school of nursing as approved and verified by a state board of nursing, and holds or is entitled to hold a diploma or degree therefrom.

Licensure, Registration and/or Certification: Valid multi-state or State of Oklahoma Registered Nurse License. Optimal Medical Therapy certification through the Heart Failure Society of America within 6 months of hire.

Work Experience: Minimum three years of clinical nursing experience, with a minimum of one year Critical Care experience, preferred.

Knowledge, Skills and Abilities: Customer-focused approach, with a broad definition of customer. Proficiency in applying nursing theories, evidence-based nursing knowledge, problem solving skills. Knowledge of pathophysiology of specific disease processes. Understanding of heart failure treatment modalities and rationales including: Guideline Directed Medical Therapy, therapeutic devices, remote monitoring devices, pre and post treatment care management of patients, and recognition of signs and symptoms of worsening heart failure. Understanding of current clinical practice guidelines for heart failure treatment plan (i.e., AHA/ACC/HFSA expectations). Effective skills in collaboration, coaching, mentoring, listening, participative decision-making, negotiating and team building. Effective verbal and written communication skills, tact and positive demeanor. Effective computer skills (i.e., Excel, Word, Power Point, etc.). Ability and desire to establish and maintain rapport with physicians and organizational personnel. Outcomes driven decision making. Ability to plan, develop, implement and administer health related programs. Ability to work autonomously, establishing benchmarks for success, and reporting these. Demonstrates understanding of and commitment to the organization's mission and values.

Essential Functions and Responsibilities: Concurrently review the multidisciplinary care of emergent and acute care patients with a diagnosis/initial presentation of heart failure according to the most recent AHA/ACC/HFSA Guidelines for Management of Heart Failure. Perform a socio-economic evaluation related to medication therapies and payor sources to optimize compliance. Establish patient relationships by assessing the patient/familys understanding of the plan of care, medical terminology, disease process, medications, and assisting with adjustment to diagnosis and adherence to the treatment plan. Assess if each patient has been provided appropriate education materials/supplies and communicates with the clinical staff/managers/case management/social work as needed to optimally serve all clients. Perform nursing practices proficiently by maintaining accurate documentation, using appropriate channels of referral and working effectively with other health care team members. Oversee continuity of care and remains a point of contact throughout the patient's hospitalization. Coordinates heart failure post discharge activities. Participates in support activities for the heart failure patient through education, caregiver support, and possibly end of life care discussions. Actively collaborates with the clinical staff and the patient/representative during the hospitalization. Participates in heart failure support activities, including staff training being available for post discharge questions/concerns of care coordination. Actively participates in the heart failure service-line's patient care activities, including committees supporting the heart failure program process improvement, program workgroups, and the Heart Failure clinic. Contributes to efforts to ensure quality services are provided on a timely basis and in a cost-effective manner.

Decision Making: The carrying out of non-routine procedures under constantly changing conditions, in conformance with general instructions from supervisor.

Working Relationship: Coordinates activities of others (does not supervise). Works directly with patients and/or customers. Works with internal and external customers via telephone or face to face interaction. Works with other healthcare professionals and staff. Works frequently with individuals at Director level or above.
