The Hampden County Sheriff's Office is looking for empathetic and detail-oriented Care Coordinators to work with the Section 35 substance use disorder clients in our Stonybrook Stabilization & Treatment Center program. The Care Coordinator is responsible for developing/facilitating of educational and treatment groups, and aftercare planning. The Care Coordinator establishes and maintains a counseling relationship with clients; assesses and analyzes information pertinent to client needs, program, and community concerns; maintains relevant information and submits reports as necessary.

Work Schedule: Tuesday- Saturday; 1 p.m - 9 p.m.

Sunday - Thursday; 1 p.m - 9 p.m.

Requirements

  • At least two (2) years of clinical experience in the field of substance addiction treatment.
  • High School Diploma or equivalent required; LADC/CADC preferred.
  • Ability to establish and facilitate a therapeutic rapport with clients, engage in effective individual counseling and intervention.
  • Strong observation skills, and ability to make appropriate treatment safety, community referrals based on knowledge of clinical theories.
  • Strong written and verbal communication skills. Ability to prepare effective, concise, and accurate written reports and discharge plans.
  • Bilingual skills in speaking, writing, and reading English and Spanish are preferred.


Company Information
The Stonybrook Stabilization & Treatment Center is not your average substance use disorder treatment facility. Created by Sheriff Nick Cocchi in 2018 to help people battling addiction, the opening of the facility more than doubled the number of treatment beds in the western half of Massachusetts.

Although the facility is operated under the umbrella of the Hampden County Sheriff's Office, it is a standalone facility with specially-trained treatment staff.

The Stonybrook Stabilization and Treatment Center provides evidence-guided, trauma-informed, and family-focused medically-managed withdrawal and post-withdrawal care in Ludlow, Massachusetts. Psycho-educational programs draw from proven models with SUD & CJ participants, scaled for appropriate engagement in the weeks following withdrawal.

The clients are admitted to the facility under the Commonwealth's Section 35 law, which provides a way to commit someone for treatment under certain conditions relative to their substance use. The commitment lasts from 30 to 90 days, depending upon the outcome of regular reviews but our average length of stay is around 50 days, which provides a solid amount of time to adequately prepare someone for success in their journey toward long-term recovery.
