Staff Nurse (LPN / RN) - Assertive Community Treatment (ACT)

A leading health services organization in New York City is seeking multiple licensed Registered Nurses (RNs) or Licensed Practical Nurses (LPNs) to join their expanding Assertive Community Treatment (ACT) team. In this role, the Staff Nurse (LPN / RN) will be responsible for providing medication management, health assessment, counseling, and psychoeducation to promote recovery and wellness, with at least 80% of services delivered in the community.

About the Opportunity:

  • Start Date: ASAP
  • Schedule: Monday to Friday
  • Hours: 9 AM to 5 PM (Full or part-time)
  • Setting: FACT and ACT team
  • Patient Population: Adults (18+)
  • Patient Volume: 5-8 clients per day
  • EMR: Awards

Responsibilities:

The Staff Nurse (LPN / RN) will:

  • Manage and administer medications (oral and intramuscular)
  • Monitor medication side effects and adherence
  • Perform phlebotomy and coordinate health care services
  • Provide psychoeducation about health and medications
  • Assess consumers' mental statuses and rotate on the team's after-hour emergency line
  • Complete progress notes, nursing assessments, incident reports, and other documentation
  • Coordinate care with prescribers, family, and community service providers
  • Participate in team and clinical meetings
  • Use motivational interviewing and trauma-informed approaches

Qualifications:

  • 3+ years of experience in a Psychiatric Nursing, Substance Use Treatment, and/or Social Services role/setting
  • NYS Licensed Practical Nurse (LPN) and/or licensed Registered Nurse (RN) license
  • At least three years of experience in psychiatric nursing, substance use treatment, or social service settings
  • Knowledge of Community Treatment and Support Services
  • Computer savvy
  • Solid assessment, clinical, and documentation skills
  • Patient oriented
