RN Integrated Care Manager at Sentara Healthcare
Minimum Requirements:
Experience: 18 months of recent RN Case Management experience.
Facility supported: Members of the team are required to work assigned shifts in all units within the case management specialty. Required to float in 2 hospitals throughout the Blue Ridge and/or Northern Virginia region.
Must be within 75 miles and 90 minutes of two of the following hospitals:
Sentara Martha Jefferson: Charlottesville, VA
Sentara Rockingham Memorial Hospital: Harrisonnburg, VA
Sentara Northern Virginia Medical Center: Woodbridge, VA
Operational hours = 8:00 AM - 4:30 PM 7 days per week
Flexi - Hours: The team member must self-schedule at least 48 hours of prescheduled time per calendar month in API. Of these prescheduled hours, 16 hours are to include availability to be scheduled on the weekend. (Prescheduled hours: Defined as clinical hours scheduled 4 weeks in advance of the current date.) Education and orientation hours will not count toward prescheduled or work hours requirements.
This position has a high degree of FLEXIBILITY for hours, and shifts, and includes Regional and/or local TRAVEL, and more pay incentives.
As an Inpatient Care Manager, you are responsible for the provision of medical social work services to patients receiving care in the hospital setting. Assesses the social, psychological, cultural, environmental, and financial situations, as well as the disposition needs of each referred patient. Collaborates with patients, families, healthcare team members, and community organizations, and uses strong advocacy skills to identify needs and resources in the hospital and community.
Develop and evaluates effective transition plans taking into consideration the client's diagnosis, prognosis course of treatment, past and present services, short-term and long-term goals, provider options, and/or available health care benefits as appropriate to the continuum of care. Job Summary
Responsible and accountable for the provision and facilitation of comprehensive care management, services and quality outcomes for patients across the healthcare continuum, primary focus on patient transition to the appropriate level of care while facilitating clinical appropriateness and LOS. Directly support those in hospital patients identified in need of comprehensive case management. Promotes effective utilization and monitoring of health services, collaborates and communicates with the healthcare team and patient/caregiver to manage care and transitions. Develops and/or implements a comprehensive care plan based on assessment and evaluation to include physical, mental and psychosocial needs of the inpatient/caregiver. Functions in one of the following practice settings: Hospitals Only
18 months Nursing experience required.
BLS within 90 days of hire.
BSN is preferred. All Registered Nurses who do not have their BSN will be required to sign a BSN agreement committing to successfully obtain their BSN within 5 years of hire.
Case Management (ACM, CCM, CCCTM, CMAC or CMGT-BC) certification required within one year of eligibility.
1 year acute care case management experience preferred.

N-2YR - RN-Associate's Degree (Required), N-3YR - RN-Diploma (Non-degree) (Required), N-4YR - RN-Bachelor's Level Degree (Required), N-6YR - RN-Master's Level Degree (Required)
Basic Life Support (BLS) - Certification - American Heart Association (AHA) RQI, Registered Nurse (RN) Single State - Nursing License - North Carolina, Registered Nurse (RN) Single State - Nursing License - Virginia Department of Health Professionals (VADHP), Registered Nurse License (RN) - Nursing License - Compact/Multi-State License
