General Position Summary: The Fitness Center Coordinator (FCC) works as part of the patient care team and assists the providers and staff in the provision of team-based care that involves nutrition, fitness, and overall wellness. The FCC provides limited direct patient care services and performs duties that enhance provider established care plans. Under the direct supervision of the Chief Nursing Officer (CNO) and with the clinical support of the provider team, the FCC plays a critical role in the delivery of quality, patient and staff centered care within the context of a federally qualified community health center.

Essential Duties and Responsibilities:

Patient/Staff Relations:

1. Reviews complete medical history from patient, and/or family, and/or previous medical record, etc. prior to a patient using the fitness center. Staff have the option to decline a comprehensive assessment provided they sign any required opt-out documentation.

2. Assesses readiness to change and confers with the individual regarding their health and nutritional/wellness-oriented goals.

3. Explains related wellness plan to patients and families as appropriate.

4. Teaches and counsels patients, staff, and families in a culturally sensitive manner.

Staff Relations/Team Building:

1. Consults with providers and other members of the health care team as necessary. Acts to facilitate communication between the members of the health care team including clinical and non-clinical staff.

2. Performs other necessary duties as required by Johnson Health Center to meet the goal of providing primary health care services.

Clinical Affiliated Duties:

1. Conducts assessments and evaluations of individual's physical abilities, health status, and exercise goals.

2. Designs and implements personalized activity and exercise programs tailored to each patient/staff member's needs, goals, and abilities as requested and/or required.

3. Instructs and educates patients/staff on proper exercise techniques and safety precautions.

4. Monitors patients' and staff members' progress and makes necessary adjustments to their exercise plans.

5. Collaborates with healthcare professionals such as physicians and providers to provide comprehensive care for patients and recommendations to patients/staff members regarding healthy activity habits to manage diseases such as diabetes and hypertension.

6. Promotes healthy eating habits by collaborating with the Nutrition & Wellness Coordinator (NWC) to provide patients/staff with education on leading healthy lifestyles and the corresponding benefits.

7. Creates exercise modifications for patients based on changing medical or physical needs as discussed with the provider team.

8. Collaborates with the NWC to oversee dietary, nutritional, weight management, and epidemiological research activities to enhance patient/staff outcomes.

9. Keeps accurate and detailed records of clients' assessments, exercise plans, and progress.

10. Physical attendance is an essential element of the job and necessary to perform the essential functions of the Fitness Center Coordinator.

Administrative Duties:

1. Maintains all patient records to comply with required standards, reviews recordsregularly and obtains physician/provider signatures where necessary.

2. Coordinates all necessary maintenance of fitness center equipment and ensures scheduled maintenance occurs per warranty and/or recommendations of contracted maintenance group.

3. Ensures fitness center remains organized and all weights and other equipment is stored in the appropriate designated spaces.

4. Ensures fitness center equipment is cleaned appropriately and all necessary cleaning supplies remain stocked and available.

Performance Improvement Activities:

1. Attends all staff meetings and participates in the Johnson Health Center QI program.

Safety/Infection Control Activities:

1. Attends all mandatory OSHA infection control trainings and follows general OSHA guidelines for infection control, including "Universal Precautions."

2. Abides by all Johnson Health Center infection control policies, including acute response measures in the setting of a public health concern or disease outbreak.

Other Functions:

1. Staff will abide by the Code of Conduct as documented in the Corporate Compliance Manual.

2. Must demonstrate a personal and professional commitment to Johnson Health Center and its mission.

3. Treats all patients and staff with dignity and respect, mindful of the cultural differences of the diverse population we serve.

4. Assures that any job functions that are added or changed by administration to meet the needs of the organization are performed accurately, completely, and efficiently.

5. Keeps professional licenses and certifications required for the position current and up to date. Obtains all continuing education required for license renewal.

6. Complies with all JHC policies and regulatory requirements.

Qualifications:

1. Bachelor's degree in Exercise Physiology or related field.

2. Certification as an Exercise Physiologist from a recognized organization (e.g. ACSM, NSCA)

3. Previous experience working as an Exercise Physiologist.

4. Strong knowledge of human anatomy and physiology

5. Excellent communication and interpersonal skills

6. Ability to work with individuals of all ages and fitness levels

7. Attention to detail and strong organizational skills 8. Ability to analyze and interpret data related to exercise physiology

9. Proficient in using fitness assessment equipment and software

10. CPR and First Aid certification

Physical Demand and Working Environment:

Fast paced office setting with travel to other offices often. Lifting and/or exerting force up to 50 pounds occasionally, with frequently moving of objects. Work requires speaking, sitting, bending, walking, standing, hearing, stooping, kneeling, and repetitive motion with certain activities. 8 hours of constant computer usage. OSHA high risk position.
