Develop one to one professional relationship with residents and families as needed for counseling. Assess, plan, and document residents discharge need. Document the social service of the care plan for the resident. Document the social service component of the comprehensive care plan for each in a timely manner. Refer residents to social, health and community resources and complete accurate document. Coordinate with family and community resources as needed. Contact the home health if resident go back to community. Conduct the resident council meeting. Participate in residents care plan meeting. Do social /Activity Charting for Annual, Quarterly assessment. Perform the one to one and group activities. Schedule indoor and outdoor activity. Assist residents with activities - Play Bingo, sing a song, coloring, watch movie, group exercise, Outing. Plan schedule room visits and in room activities for residents unable to leave the room. communicate and work with other department for residents need.
