Description: Job Summary:Under immediate supervision, provides a variety of functions maintains and distribute medical records in the service area, performs clerical duties which include filing, completing chart requests/appointments, lists reviews. Reviews and monitors the electronic document management system and the records contained within for appropriate image quality and indexing specificity. Ensures the quality and integrity of the scanning and indexing functions of the electronic medical record. This position serves as the Subject Matter Expert for the scanning and indexing function within the department.Essential Responsibilities:Ability to perform job function and make decisions without direct supervision. Compile, maintain and report performance statistics for scanning and indexing staff to department management. Assist in developing, preparing, presents and updating training materials; orients, trains, and provides in service education for scanning and indexing personnel. Identify issues, research problems and bring issues to resolution. Independently determine and make necessary corrections and adjustments, as appropriate. Knowledge of medical record format and content of all patient records. Cross-references patient information to ensure it matches scanned document and confirms page count. Determines if scanned image requires further research and forwards scanned image to Research work queue. Indexes the scanned images and enters the required patient data elements. Retrieves and prioritizes batches of scanned images, scans images and determines if document needs to be re-scanned. Using data entry and performs the necessary indexing operations to link the scanned image with the encounter. Ability to identify nonstandard forms, medical record formats and determine action required. Correctly inserts document into the scanner and ensure document is correctly scanned. Photo copies documents that are poor quality and image before scanning. File, pull, maintain the members/patients medical records using the alpha/numeric filing system and according to the chart format. Sorts documents and preps them for scanning/indexing. Respond to requests for appointments and chart review. Establishes and maintains courteous and cooperative relations with the public and other staff. Prepares and pulls appointment schedule/add ons, lists, for delivery. Checks incoming and outgoing charts for accuracy and missing forms; locates missing documents and records; reconciles illegible or missing data; reactivates and activates charts. Utilizes various, office equipment, fax machines, multi phone lines, PCs, copiers, printers, scanners, etc Repairs charts and documents by replacing folders and chart thinning. May be requested to perform duties other areas in the department and in the mailroom department. Train and orient new or less experienced personnel and serve as resource person to coworkers by answering procedure questions and assisting in problem solving. The actual depth of responsibility for the clerk varies at different medical centers. The supervision of the clerks is also a variable in each department Upholds Kaiser Permanentes Policies and Procedures, Principles of Responsibilities and applicable state, federal and local laws. Basic Qualifications:ExperienceMust pass Medical Terminology Test. Alpha numeric file/Test/File clerk test/chartroom clerk test Ability to compose and write basic correspondence. Ability to clearly, read, write and thoroughly understand English. 2 years Health records clerical experience required. Must be able to tolerate standing, stooping, bending and reaching for significant periods of time. Ability to push or lift up to 50 pounds. Ability to push a cart weighing up to 100 lbs required. Must be able to work in a Labor/Management Partnership environment. Certificate in medical terminology preferred. Working knowledge of personal computers with the ability to navigate in a MS Windows environment using mouse and keyboard skills. Strong computer skills Typing skills of 25 wpm.EducationHigh school graduate or equivalent with GED.License, Certification, RegistrationN/AAdditional Requirements:Preferred Qualifications:Proof completion of 4 HIT courses in an accredited HIT program preferred. Associate Degree preferred. Competent knowledge and understanding of anatomy and physiology preferred. Excellent verbal and written skills. Good interpersonal relationship building skills.
