Wilshire is offering a $5,000 Sign on Bonus for Full-Time RN Positions
We re looking for a motivated professional to join our team of Registered Nurses!
Under the direction and supervision of the Clinical Manager, the Registered Nurse Case Manager is responsible for the comprehensive nursing care of assigned patients within a region. Duties include, seeing 3-5 patients per day, medication management, symptom management, psycho-social support, supervising LVNs and Home Health Aides assigned to the team, and collaborating with an interdisciplinary team. The RN Case manager must be able to work autonomously, have good time management skills, and be able to communicate and delegate concerns or needs professionally.
Essential Functions of the Job Summary:
- Conducts initial evaluation visit, and submits completed paperwork in a timely manner.
- Responsible for the admission process and orientation of patients.
- Completes full body assessment on each patient during initial visit.
- Assesses the total needs of the patient and develops nursing care plans to meet those needs.
- Initiates appropriate preventive and rehabilitative nursing procedures.
- Initiates and revises the plan of care as needed, clarifying new orders and discontinued orders.
- Prepares clinical and progress notes.
- Responsible for accurate observations, evaluations and reporting of symptoms, reactions and progress of patients to the physician
and Clinical Supervisor, and others participating in the care and treatment of the patients
- Multiple shifts available: Mon-Friday 8am -5pm primarily with some weekend rotation
- Occasional back up call for NOC nurse.
- May have to travel outside of assigned service area on occasion.
Qualifications and Requirements:
- Current licensure as a Registered Nurse in the State of California.
- One or more years of experience as a Registered Nurse is required; Med Surg experience preferred.
- Graduation from an approved school of professional nursing, BSN preferred.
- Demonstrated knowledge of and ability to apply basic principles of nursing care.
- Experience and/or education in the area of the aged, chronically ill, and/or psychiatric nursing.
