Position Overview: We are seeking a compassionate and experienced Hospice Registered Nurse (RN) Case Manager to join our dedicated team. The RN Case Manager will be responsible for coordinating and directing nursing services for patients in need of hospice care, utilizing advanced assessment, teaching, and decision-making skills. This role involves independent management of a diverse patient population and ensuring the delivery of patient-centered care in accordance with hospice regulations. Responsibilities:

Review patient records, establish care plans based on best practices.
Organize schedule to minimize travel time between visits.
Obtain MD orders, implement care plans accurately.
Perform comprehensive patient assessments, adjust care plans as needed.
Order supplies, durable medical equipment according to hospice criteria.
Customize care plans for spiritual, psychosocial, emotional, and physical needs.
Evaluate care plan effectiveness, adjust interventions collaboratively.
Supervise LNAs in compliance with regulations.
Document data, interventions accurately in patient charts.
Complete admissions, recertifications accurately and timely.
Conduct HIS outcome assessments precisely.
Participate in QAPI projects, adhere to clinical policies.
Attend required in-services, state trainings, competencies.
Communicate effectively with team, healthcare professionals.
Utilize case management concepts in practice.
Involve patients, families in plan of care, IDG meetings.
Actively contribute in IDG meetings

Requirements:

Education and Licensure:

Current State of New Hampshire or Compact State licensure/registration in Nursing.
Graduation from an approved nursing curriculum by the state and designated nursing governing board.

Experience and Skills:

Minimum of two years of med/oncology experience preferred.
Minimum of one year of hospice experience preferred.
Continuing education to meet state requirements.
Comfortable with technology and able to work independently.
Strong communication, problem-solving, and decision-making skills.
Ability to tolerate prolonged sitting, occasional inclement weather driving, and use personal cell phone for job-related tasks.
