null ** Must have hospice experience 1 yr or greater** Facility location: 2673 Davisson Run Road Clarksburg, WV 26301 Distance/Counties covered: Harrison, Barbour, Upshur, Lewis, Taylor, Doddridge, Wetzel. It is our Clarksburg location and they cover 7 counties but will be a smaller area and work between 2 RNs. # patients treated per day: 5-6 visits in 8 hours. Certs Required BLS, WV license EMR EPIC Weekend Requirement None Holiday requirement none Scrub Color/Uniform Royal Blue Outpatient and Inpatient? Hospice Pharmacy 24/7 n/a Shifts M-f 8-430 What areas are they required to cover? Above counties specific orientation Epic and with another PT Common diagnosis / Types of patients Terminally ill patients in their homes or nursing facilities. Cancer, heart disease, lung disease, dementia all end stage diseases.
