The Ohana Crisis Stabilization and Intellectual & Developmental Disabilities Program is a group home in a community setting for adolescents with intellectual and developmental disabilities. If you are looking for an opportunity to make a difference and change lives for the better, then this job could be for you!The Transition Specialist is responsible for assisting individuals transitioning from institutional settings back into the community. Performs a variety of duties including counseling, assisting and supporting adolescents/young adults with psychiatric, developmental and/or social-behavioral challenges to maximize their independence through skill development. Will collaborate with the Case Management Organization (CMO), Division of Child Protection and Permanency (DCPP), coworkers and the general public to implement strategies that will encourage a positive outlook. Position will facilitate support groups, such as psycho-educational, to help develop social, emotional and independent living skills. May serve as planning partners with the youth to assist in identifying and making necessary connections. Primary duties and responsibilities include:Conduct family orientation in the first 24 hours of admissionReview and sign all required documents and consents within the first 48 hours of intakeReview intake information to determine if the medical examination has been completed 30 days before intakeProvide on-call and after-hours support on a rotating basis for after-hour emergenciesOrganize psychiatric evaluation as neededCollaborative relationships with system partners including CMO and DCP&PKnowledge of useful resourcesOrganizing monthly goals for the youth to lend to a successful transition taking placeFollow up daily with the program staff to obtain information to build the youth s transition planContact parents/caregivers at minimum twice per week to discuss the status of the youth s transitionsProvide on-site family psychoeducational activitiesOrganize discharge plan monthly and comprehensive treatmentGathering information from other team members: nurses, directors, behaviorist, clinical and residential counselorsUpdating the youth s progress notes with Children s System of Care (CSOC) s CYBERHours:Position is full time and includes the following:3.2 weeks of Paid Time Off during the first yearMedical, vision, dental and life insurance9 Agency-paid holidays403(b) employee participation and employer matchTraining opportunities provided throughout the year for both FT and PT employees$2.00 per hour shift differential for hours worked from 11pm Friday through 11pm Sunday for both FT and PT employeesTuition Reimbursement after 1 yearEmployee Assistance Program & moreEducation/Experience:Bachelor s level practitioner with 3-5 years of relevant experience with individuals who are Intellectually and/or Developmentally Disabled (IDD), or an unlicensed Master s level practitioner with 1 year of relevant experience.Legacy Treatment Services is an Equal Opportunity Employer.
