What You ll Do

Support the clinical home health and/or hospice teams to utilize a collaborative process to review, plan, monitor, and evaluate the options and services required to meet patients needs while coordinating available resources to promote quality and cost-effective outcomes. Specifically, we will look to you to collaborate to manage processes, protocols, and timeliness of all referrals to meet the comprehensive needs of the patients.

What It Takes

  • High school diploma or equivalent required
  • Current Ohio LPN license required
  • Associate degree in health information or related field preferred
  • Two years of medical records experience in a long-term care, hospital, or other related healthcare setting required
