Title: Hospice Triage RN
What Youll Do:
+ Provide support to staff and telephone interventions to maintain patients safely in their home.
+ Scheduling home visits as needed.
+ Collaborate with Physician practices and hospital staff to influence effective management of patients in their homes.
What Youll Bring:
+ Current R.N. license in the State of Maine.
+ Current CPR card required.
+ One year of recent clinical nursing experience required.
+ Hospice experience required.
