Job Summary:
The Community Health Navigator supports the social, economic and behavior health needs of vulnerable patients within Maui Health System and the communities we serve. The CH Navigator works on inter-disciplinary care teams supporting members who are experiencing complex socio-economic circumstances impacting their health and health care. They work to identify goals an enable members and their families to achieve those goals through identifying local community assets. CH Navigators are non-licensed. They are highly trained communicators and experts on how to access local community resources and social services. They are skilled in motivational interviewing and person-centered care. Specifically, the CH Navigators coordinate alternative services for patients with social and non-medical needs and where other avenues of care are more appropriate to affect optimal patient outcomes, achieve continuity and quality of care and reduce cost (Triple Aim). Patient/Family education will be a key component of success.
Essential Responsibilities:

  • Supports members and their families who are experiencing complex circumstances. Works to identify goals and enable members and their families to achieve those goals through identifying local community assets, vetting their services and making these resources accessible to patients and providers.
Basic Qualifications: Experience
  • Minimum one (1) year of experience in a health care or related field.
Education
  • Bachelors degree in a business or health care related field OR four (4) years of experience in directly related field (the following may substitute: associates degree in a business or health care related field with an additional two (2) years of directly related experience).
License, Certification, Registration
  • N/A
Additional Requirements:
  • Competent computer skills with proficiency in Microsoft Office products and ability to learn new programs to support the program.
  • Ability to demonstrate flexibility and adaptability when faced with internal or external barriers, or when faced with differing points of view.
  • Ability to anticipate next steps, be proactive and collaborate with coworkers and stakeholders.
  • Ability to use integrated technology platforms and virtual care coordination tools
  • Ability to use good judgment and problem solving skills, able to effectively respond to difficult situations and resolve conflicts.
  • Ability to work independently in a fast-paced environment.
  • Excellent written and verbal communication skills.
  • Ability to complete Motivation Interviewing Certification.
  • Ability to organize and prioritize workload; effective time and project management skills in achieving program initiatives and priorities.
  • Ability to successfully complete tasks with little or no supervision.
  • Ability to anticipate patient and team member needs.
Preferred Qualifications:
  • Preferred experience in the community health care setting.
  • Preferred experience as a health coach and/or community health care worker.
  • Community health worker certification.
  • Bachelors or Masters degree in a business or health care related field.
