Job Summary:
Menorah Park, located in Syracuse, New York, is seeking a Full Time LPN Care Coordinator to join our team. This individual will play a vital role in coordinating and managing the care of our residents, ensuring they receive the highest quality of care. The LPN Care Coordinator will work closely with our healthcare team to develop personalized care plans and ensure that all medical needs are met. This is a fast-paced position that requires excellent communication and organizational skills.

Compensation & Benefits:
This is a full-time, hourly position with a pay rate of $25 per hour. Employees will be paid on a weekly basis. Menorah Park offers a comprehensive benefits package, including medical, dental, and vision insurance, as well as paid time off and a retirement plan.

Responsibilities:

  • Plans safe, economical, efficient, and therapeutic care.
  • Coordinate care received by the client.
  • Plans and assists in reinforcing client and family teaching and in the supervision of procedures as taught.
  • Provides direct client care.
  • Implements all phases of the nursing process.
  • Documents all information related to client care.
  • Recognizes changes in client status and reports to appropriate persons.
  • Acts as a liaison between client, family and physician.
  • Maintains communication and coordination among all services provided.
  • Assists in planning for discharge of clients when appropriate.
  • Maintains timely physician orders. Verbal orders are reduced to writing and are co-signed by the physician within 14 days.
  • Maintains written and verbal communication with physician regarding client status.
  • Utilizes paraprofessional personnel to assist the client as appropriate and ordered.
  • Performs aide supervision according to policy.
  • Provides the side with demonstration of assigned duties and has aide re-demonstrate procedure(s).
  • Communicates effectively regarding matters impacting the agency.
  • Functions within the policies and procedures of the agency, adheres to the mission and philosophy of the agency.
  • Collaborates with supervisors to improve performance.
  • Recognizes the need to provide information to other personnel as required for efficient functioning of the agency.
  • Participates in activities beneficial to the growth and development of the agency, Functions in a professional and efficient manner.
  • Maintains professional skills and continues to grow professionally.
  • Operates within the scope of practice for a Licensed Practical Nurse as defined by the Nurses Practice Act.
  • Responsible for medication cart audits and chart audits
  • oversee schedule and on call rotation for staffing.
  • ordering of medications and refills

Requirements:
- Active LPN license in the state of New York
- Minimum of 1 year of experience in a long-term care or healthcare setting
- Strong communication and interpersonal skills
- Ability to work independently and efficiently in a fast-paced environment
- Proficient in electronic medical records systems
- Excellent organizational and time-management skills
- Ability to prioritize and multitask effectively
- Demonstrated empathy, compassion, and patience towards residents and their families
