Home Health LPN Needed - Fort Wayne, IN
13 weeks - Day 5x8-Hour (08:00 - 17:00)
* $46 per hour
* $51 overtime rate
Years of experience REQ: Minimum 1 full year Adult Home Health Experience or 1 full year Hospice experience depending on the specialty
Charting: All professions must be proficient with HomeCare HomeBase (HCHB) and used it within the last 6 months. If they have used it outside of the last 6 months they may apply but will be required to take a retraining course during compliance.
Overview:
We are seeking a dedicated Licensed Practical Nurse to join our healthcare team. The ideal candidate will have experience working with individuals with developmental disabilities and possess a strong knowledge of medical standards and procedures.
Responsibilities:
- Provide direct patient care in various settings including acute care, behavioral health, ICU, and ophthalmology
- Perform diagnostic evaluations and assist in treatment planning
