Role Description
This is a full-time hybrid role for the Palliative Home/outpatient Program, Nurse Practitioner Palliative Care located in Charleston, South Carolina Metropolitan Area that allows some work from home as well. Visits in home, in person, virtually and in clinic setting one day per week.
Qualifications
- Master's Degree in Nursing, with a focus on Adult-Gerontology, Acute Care (preferred), Palliative Care, Hospice or Family Practice
- Current certification and license as an Advanced Practice Registered Nurse (APRN) in South Carolina
- Current certification in Basic Life Support (BLS) for HealthCare Providers
- Minimum of three years of experience in patient care, palliative care, or acute care
- Good communication and relational skills with a sensitivity towards the needs of patients and their families
- Excellent attention to detail, problem-solving abilities, and the ability to manage multiple tasks
- Excellent organizational, documentation, and electronic medical record (EMR) skills
- Ability to work independently and coordinate with interdisciplinary teams
In addition, the candidate should possess excellent knowledge of palliative care, pain management, pharmacology, and medical treatment options for adult and geriatric patients and have experience in wound care, care for central lines, and superior clinical assessment skills and practice-based experience of quality improvement processes.
