Job Overview: Blue Zones Health, a Medical Group dedicated to providing care through lifestyle medicine for Medicare and commercial patients, is seeking a proactive and engaging Community Connection Representative. This remote role focuses on engaging with existing members, following up on leads generated by the Community Activation Representative and Blue Zones Health, and ensuring a seamless experience for our patients. The ideal candidate will excel in communication and customer service, ensuring members are well-informed and supported.


Key Responsibilities:


  • Member Engagement:
  • Survey Blue Zones Health members based on provider, health plan, add/term status, and program involvement.
  • Reach out to leads collected by the Community Activation Representative.
  • Answer member questions and document discussion summaries in a CRM.
  • Drive attendance to local events through effective communication and follow-up.
  • Coordination and Support:
  • Coordinate with Social Support and Community Activation teams to provide necessary assistance.
  • Introduce patients to brokers for Medicare Advantage or health insurance enrollment.
  • Facilitate three-way calls with health plans and members for IPA changes.
  • Support members in scheduling appointments with Blue Zones Health in network providers.
  • Communication:
  • Communicate health plan changes to Blue Zones Health members, including plans no longer accepted and new plans being added.
  • Inform patients when their provider is no longer part of the Blue Zones Health network.
  • Fielding inbound calls from brokers and patients to answer questions and provide basic information about Blue Zones Health and it s network.
  • Feedback and Reporting:
  • Provide valuable feedback to the leadership team regarding patient experiences with Blue Zones Health clinicians.
  • Document all member interactions and activities accurately in the CRM.
  • Report on daily, weekly, and monthly engagement metrics.


Ideal Qualifications:


  • Education: Bachelor s degree in Communications, Marketing, Healthcare Administration, or equivalent experience.
  • Experience:
  • 3+ years of experience in customer service, member engagement, or healthcare support roles.
  • Experience working with Medicare and commercial healthcare plans is highly desirable.
  • Skills and Abilities:
  • Excellent verbal and written communication skills.
  • Strong interpersonal skills with the ability to build and maintain relationships with members.
  • Proficiency in using CRM software and Microsoft Office Suite.
  • Strong organizational skills and attention to detail.
  • Ability to work independently in a remote setting.
  • Empathy and a strong commitment to improving patient experiences.
