About the Role

As a Community Health Worker, you play a critical role on the Hopscotch care team, working closely with a physician and others on the team to serve our patients, especially in the community. Building trust with patients and helping to enable positive outcomes and experience are two focus areas for this role.

This role is based in our new clinic location in Hudson, North Carolina and will start in September 2024.
What You'll Do

In this role, you will help our patients live better lives - Be their main contact, understand their needs, challenges, and barriers and work with the care team to develop solutions and coordinate care to meet their needs:

  • Develop trusted relationships with patients by providing support and advocacy to help achieve health goals
  • Help patients access resources in the community to address social and economic needs
  • Identify high priority needs to ensure patient satisfaction and positive outcomes, and work with the care team to develop plans and take action to address those needs
  • Facilitate communication across all parties (Patient, Hopscotch team, community providers, family, etc)
You will also support end-to-end care needs for new and existing patients in the Hopscotch care model across care settings:
  • Develop a care plan including objective goals and work across the care team to progress towards those goals
  • Meet with patients where it is convenient for them (e.g., Hopscotch Primary Care center, facility, home, community setting)
  • Partner with the care team to help patients and their families prepare for the patient leaving the hospital, emergency department, or skilled nursing facility
  • Drive engagement with patients (e.g., completed specialty appointments, adherence to post-discharge plans, adherence to medication regimen), which may include accompaniment to appointments
  • Assist application completion to access eligible benefits
  • Conduct wellness checks and enable remote monitoring and virtual care as needed
  • Help navigate managed care plans by understanding details and resources, and ensuring mindful utilization by working with the care team and patients to develop recommendations
  • Maintain documentation for patient enrollment and program participation based on compliance and business needs
Additional requirements include:
  • The ability to transport oneself to various locations in and around the Asheville, NC community, including patient homes, paying attention to travel and compliance policies
  • Compliance with all internal and external requirements, including legal, regulatory and HR
  • Support for compliance audits, internal and external, as needed
  • As needed, represent Hopscotch Primary Care in the community and provides community-based education, engagement, and participation in events such as health fairs/conferences
About You

You would be a great fit for this position if you have:

  • Basic understanding of the healthcare system and local knowledge of the community
  • A track record of being organized, reliable and trustworthy in your interactions
Previous experience in healthcare is not required but you must also have:
  • Proficient PC skills
  • Fluency in language that is commonly spoken in the community, where necessary
  • Ability to travel locally
  • US work authorization
