GBMC Health Partners (HP) and Gilchrist Elder Medical Care (EMC) are seeking a Community Health Worker (CHW) to serve as a frontline public health worker and trusted liaison between underserved communities in Baltimore and the healthcare systems. The CHW will work to ensure increased access to primary care and Social Drivers of Health (SDH) and will report to the Community Outreach Manager. Responsibilities include promoting primary care programs, providing literature to partner organizations, attending events, conducting health risk and SDH assessments, enrolling new patients and scheduling community members in primary care, referring to SDOH resources, and advocating community member health needs. Support community members in adopting healthier behaviors.

Education

High School or equivalent with an associate or bachelor's Degree preferred

Experience

  • Preferred qualifications for this position include at least 1 to 2 years of experience working in a community-based setting and experience in a multicultural environment.

Knowledge, Skills and Abilities

  • Strong empathetic listening and communication skills including excellent verbal communication with patients and community members both on the phone or in person.
  • Demonstrate empathy, a genuine concern, and passion for helping community members and patients.
  • Excellent technical, project management, and communication skills.
  • Strong Excel, PowerPoint, and data analysis skills.
  • Proven track record of success in cultivating relationships through collaboration.
  • Strong assessment skills to identify the customer's needs, clarify information, and provide resources and information that best meet assessed needs.
  • Experience with community organizing in our service area preferred

Utilize electronic record keeping systems, computers/keyboards, and or laptops and other related equipment.

Licensures, Certifications

  • Community Health Worker Certification within two years of hire

Physical Requirements

  • Demonstrates the physical capability to drive in suburban and urban areas to visit partner organization sites, as well as to engage in community events by walking, climbing stairs, and transporting promotional materials using a cart.

Patient Population:

  • Demonstrates competency in the delivery of care and applies the knowledge to meet age-specific needs if applicable.

Principal Duties and Responsibilities

  • Conduct a Health risk/SDOH needs assessment.
  • Schedule initial primary care appointments for community members who need healthcare.
  • Provides ongoing follow-up with patients to ensure initial primary care appointments, SDOH enrollment and access.
  • Manage assigned caseload and ensure regular Follow up with patients via phone calls, and visits to other settings where patients can be found to ensure access to SDOH fulfillment.
  • Provide referrals for services to community partner agencies that offer SDOH.
  • Record patient contacts, referrals, and enrollments in the EMR and other software no later than 24 hours after patient contact.
  • Attend and Table at community partner organizational events and programs to promote enrollment in GBMC HP Baltimore or Gilchrist EMC Baltimore primary care programs.
  • Partner with GBMC RN Health Coach and medical provider at community events. Offer information and about GBMC HP Baltimore or Gilchrist EMC Baltimore primary care and SDOH program.
  • Serve as the liaison to community partner organizations to assist with planning their events and attend the organization HERC events.
  • Attend regular staff meetings, Community Partner Coalition meetings, trainings, and other meetings, as requested.
