Pay: $52,000/ year

As The Re-Entry Case Manager, you'll have a dynamic and critical role providing continuous advocacy and education around support services in the community in order to reduce recidivism, homelessness, and advocate employment. The Case Manager will seeking qualified client referrals from Essex County Sheriff's Department facilities to participate in the Pathway Home program.

Some of your critical responsibilities:

  • Participate in outreach and advocacy efforts to help inmates in Essex County both at the correctional facilities and upon release in the community.
  • Develop, maintain, and document a strong network of referral sources designed to generate positive outcomes for program participants. Connect with other providers, employers, and employment placement agencies, including housing providers, for referrals and linkages.
  • Provide pre-screen, intake, and assessment on qualified participants, and work with community partners to attain housing, employment, treatment, and other supportive services.
  • Provide day-to-day support and services for participants with a specialization in employment placement and support, homelessness prevention/rapid re-housing, and transitional supportive services.
  • Assist the prospective clients in gathering the documentation necessary for enrollment.
  • Conduct regular reviews referral log to determine applicant trends and percentages of applicants accepted to and attending the program.


The Case Manager role is about making a difference in the lives of the people we serve. You'll bring the following qualifications:
  • O ne year of experience in providing intake, outreach, or case management activities
  • Bachelor's degree preferred but not required in education, psychology, rehabilitative counseling, or related human services field
  • Demonstrated experience/work history working with corrections or re-entry field, mental health and/or substance abuse programs.
  • Proficiency in Microsoft Office as well as exceptional written and verbal communication skills
  • This position will require independent travel between worksites, thus requires a valid Driver's License and insurance coverage on a personal vehicle at all times during employment.
  • Must be able to satisfactorily pass ECSD background clearance process including attending the required week-long orientation through the correctional facility.
  • Written and spoken fluency in Spanish strongly preferred
