Competencies:

The potential employee will have the ability assess and interpret information related to population; have knowledge of the symptoms of the disability/disorder and ability to recognize exacerbation of symptoms; have a general understanding of treatment needs; knowledge of available treatment resources and appropriate use; Population-specific communication skills.

Responsibilities:

* Specialist focuses on aiding individuals with obtaining emergency, temporary, long-term and permanent housing solutions while they are receiving treatment for their mental health and/or substance use disorder.

* Specialist will help address housing needs with the goal of reducing the impact homelessness or housing instability has on an individual's recovery.

* Provide a comprehensive case management assessment that addresses life domains such as social support, treatment needs, finances, transportation, legal services and health care;

* Work within a multi-disciplinary team approach to coordinate services with family, vocational and housing specialists as well as peer supports;

* Maintain strong working relationships with Camden County treatment facilities, acute care centers, resources and stakeholders;

* Develop and implement individual recovery plans that are person-centered and include strategies for recovery; the plan shall identify priorities, desired outcomes and the strategies and resources to be used in obtaining outcomes based on the case management assessment;

* Provide training to clients on the recognition of relapse triggers and strategies to deal with avoiding a relapse;

* Care manager will determine the type and frequency of services desired by person served. The services provided will be responsive to the individual's needs;

* Use engagement skills to motivate and encourage individuals to voluntarily access services that would assist them in maintaining a healthy lifestyle;

* Advocate on behalf of the individual by providing a holistic experience;

* Document all consumer sessions and outreach rendered thoroughly and timely in the ECR;

* Participate in multi-disciplinary team meetings and individual supervisions per agency standards;

* Other duties as assigned.

Qualifications:

* Bachelor's Degree in Psychology, Sociology or related field

* Two (2) years' experience working with high needs individuals within a behavioral health setting

Programs funded through SAMHSA must follow federal guidelines for a drug free workplace.
