What youll do

We will look to you to represent our life plan community and our organization to the greater community. Youll work with case managers, social workers, and discharge planners to generate and facilitate new referrals. Patients and families will look to you also to help evaluate their needs for rehab and long term care services.

What it takes

  • Bachelors degree in business, marketing or nursing
  • Two years experience in a long term care facilty or in healthcare
  • Strong knowledge of the full continuum of care and eligibility requirements for home health, hospice, skilled nursing, and assisted living required
  • Must possess a valid drivers license and acceptable driving record
