No Required/Preferred Years of experience REQ: 3 years of RN experience First-timers accepted: No- Must have Travel Experience Certs/Lic REQ: BLS Education REQ: BSN Certs Preferred: Case Management certification Experience Preferred: Prior leadership experience and Epic Experience Looking for a Nurse Case Manager (NCM) I reports to the Manager of Case Management and Care Transitions. Under guidance of the Nurse Case Manager Team Leader, the Nurse Case Manager I functions as a member of the clinical team. This role coordinates a plan of care to achieve clinical outcomes and promotes a safe and timely discharge. This position manages the plan of care throughout the care continuum to ensure the efficient movement of the patient through the hospital system. The Nurse Case Manager I serves as a clinical expert related to discharge planning, resource utilization, and case management. Misc Open to accommodating block schedule? Approval prior to assignment, but not guaranteed. Radius rule? Yes, 60 miles. Are locals accepted? Yes, on a case-by-case basis at 10% less than the posted bill rate Pending License accepted: SC or compact state license in hand at time of start RTO Restrictions: All RTO must be included at time of submission. Approval will be based on facility's needs. Guaranteed Hours: Facility allowed to call off 1 shift per 2 weeks Hospital Highlights Type of Facility: Teaching Hospital Magnet / Teaching / Level Trauma: Magnet, Teaching and Level 1 Trauma Total Staffed Beds: 865 Scrub Color: Business casual attire Charting: Epic Parking Cost: Free parking during the week at our Hagood lot which is about a half mile away from campus. Shuttles from Hagood are provided free of charge Monday -Friday but not on weekends and holidays. An optional after-hours parking pass for $20 is available, this allows parking on campus after 4:30pm Monday-Friday, weekend and holidays.
