Job Title/Position: Intake Coordinator
Reports to: Branch Clinical Director
Position Summary: Primary function is to facilitate the intake referral process, coordinate care with the interdisciplinary team and the referral source.
ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES
Patient Care:
- Receive and appropriately document new referrals in a timely manner
- Participate in coordinating care with Nurse Supervisors and patient interdisciplinary team
- Respond to agency and patient needs in a professional and creative manner
- Demonstrate commitment, professional growth and competency
- Promote Agency philosophy and administrative policies to ensure quality of care Perform on call rotation as assigned per on-call schedule.
Communication:- Provide effective communication to patients, staff members, other health care professionals and referral sources
POSITION QUALIFICATIONS1. Graduate of an accredited school of vocational nursing
2. Current Texas State license as a Licensed Vocational Nurse
3. One year experience as a Licensed Vocational Nurse, home health preferred
4. Nursing skills as defined as generally accepted standards of practice.
5. Proof of Hepatitis profile
6. Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order.
SKILLS REQUIRED1. Good interpersonal skills.
2. Works in a routine office environment. Noise level may be moderately high, ability to work a flexible schedule and extended hours. Possible exposure to toxic materials, toner, etc.
3. Requires working under some stressful conditions to meet deadlines and patient needs, and to make quick decisions. Requires excellent problem-solving skills. Organized and resourceful self-starter, strong ability to work in a team.
4. Excellent observation, verbal, written and interpersonal communication skills.
