Job Details

Essential Duties and Responsibilities:
1. Collaborate with community partners (including schools, health departments, social services, Head Start, faith-based organizations, churches, industries, and other community based organizations) for events and projects to reach and educate the community on health insurance options and changes, JHC services and the importance of social capital effects on health.
2. Respond to patient requests for resources by completing social determinants of health assessments and documenting in Electronic Health Record.
3. Interview, evaluate, and follow-up with patient and/or family to determine patient fee scale, as necessary.
4. Educate individuals and communities on how to navigate health and social service systems in a linguistically and culturally competent manner.
5. Advocate on behalf of patients to community members, legislators, the media and other professionals or organizations about patient concerns.
6. Attend community functions on behalf of JHC to conduct outreach and insurance enrollment.
7. Successfully complete state-specific as well as federal training to learn how to navigate the health insurance marketplace and become a Certified Assister.
8. Successfully complete Community Health Worker training and obtain full certification, to be renewed every three years.
9. Promote healthy lifestyle living such as planning and conducting health classes for varied audiences.
10. Distribute health related brochures and other educational materials throughout the community.
11. Maintain a resource file of available social service agencies and assistance providers in community for use in referring patients. Utilize online referral software when necessary.
12. Maintain log of individuals and organizations served and outreach activities conducted.
13. Physical attendance is an essential element of the job and necessary to perform the essential functions of the job.
14. The CHW will be responsible for documenting, tracking and ensuring resolution for all patient events for their assigned location.
15. Performs related duties as assigned by supervisor including but not limited to JHC hosted events, fundraisers and specific patient demographic outreach.

Other Functions:
1. Execute all outreach and enrollment activities in a consistent and professional manner.
2. Must be able to successfully complete all required training and become a Certified Assister for the health insurance marketplace and Medicaid.
3. Must be able to successfully complete all required training to become a Certified Community Health Worker within one year of employment.
4. Ability to work effectively with all levels of community individuals, administrative and professional personnel and to collaborate with a team to creatively anticipate and solve challenges.
5. Staff will abide by the Code of Conduct as documented in the Corporate Compliance Manual.
6. Must demonstrate a personal and professional commitment to Johnson Health Center and its mission.
7. Treats all patients and staff with dignity and respect, mindful of the cultural differences of the diverse population we serve.
8. Management may modify, add or remove any job functions as necessary or as changing organizational needs require

Qualifications:
1. Bachelor's Degree in social work, health promotion or other human services or nursing field preferred; Associates Degree in human services or related field OR Certified Community Health Worker required.
2. Knowledge of navigating health insurance marketplace and/or Medicaid preferred.
3. Must possess knowledge of a broad range of social service and assistance providers in community.
4. Demonstrated ability to work as a member of a team and work effectively with community agencies.
5. Strong interpersonal communication and relationship-building skills.
6. Cultural competency to communicate appropriately with a variety of populations.
7. Ability to use computer software systems and Internet.
8. Must have valid driver's license and reliable insured transportation required.
9. Must have acceptable driving record as demonstrated by recent printout by the Department of Motor Vehicles.

Physical Demand and Working Environment:
Fast paced office setting with travel to other offices often. Lifting and/or exerting force up to 25 pounds occasionally, with frequently moving of objects. Work requires speaking, sitting, bending, walking, standing, hearing, stooping, kneeling, and repetitive motion with certain activities. Extended hours of constant computer usage. OSHA low risk position
