**Case Manager IHC: Intensive Home Care**

Remote - Texas / Devoted Medical / Full-Time

Were on a mission to change healthcare how its paid for, delivered, experienced. We want to put people center stage, not process or profit. We are guided by a deep belief that every person on Medicare should be treated like we would treat a member of our own family: with loving care and a profound commitment to their health and well-being.

**Why Devoted:** Make a difference... a big difference. You will have the opportunity, in a unique and dynamic culture, to be a part of something incredibly meaningful. Were a disruptive startup with an inspiring mission. Learn and grow alongside our world-class team of healthcare, technology, and data leaders, dedicated to building a health plan and clinical services that care for everyone like they were our own mothers or fathers. Our team and board members have decades of experience in health care entrepreneurship, data, technology, services, and policy. With their support, we look forward to building a health plan and clinical services that will give seniors easy access to the high-quality care they deserve.

**A bit more about this role**:

We want to build a medical group around Devoteds values of treating every member like family. The most complex patients are some of the patients most poorly served by the status quo system. The intensive home care (IHC) program is an interdisciplinary team designed to holistically build a better care system for those patients.

The IHC case manager is a critical part of the IHC care team, serving as both team quarterback and empowered patient advocate, ensuring the most vulnerable patients are supported, attended to, and enabled to live their best possible life. To do so requires traditional case management roles, and some broader team management roles.

The more traditional case management pieces include: 1.) Ensuring smooth transitions of care for patients in the hospital or recently discharged, seeking to minimize readmissions (including in-person visits to local hospitals) 2.) Providing telephonic, in-facility, and/or in-home longitudinal case management support for poorly controlled chronic illness, 3.) Facilitating chronic disease management including coaching patients and remote monitoring.

Team management roles include: 1.) Working closely with community health workers to ensure that they are delivering practical support to NPs, Case Managers and Social Workers, allowing everyone to leverage their respective expertise to in addressing the needs of complex patients, 2.) Running twice-weekly member rounds to ensure the entire IHC team is focused on the most vulnerable patients, and 3.) Building relationships with key local market stakeholders (e.g. PCP groups)

In short, the case manager sews together the quilt that pieces all the fragments of the health system for the most complex patients.

Our ideal IHC case manager is caring, compassionate, solution-oriented and enthusiastic about providing an outstanding experience for Devoted Healths members. They are committed to integrity, excellence, and empowering our most complex members to confidently navigate the healthcare system and live healthier lives. They are ready to innovate, adaptable to a continuously evolving startup environment, and willing to start scrappy, working with the whole Devoted family to create a revolution in how care is delivered.
